Does our doctor need to sign anything?
  • No there is nothing to sign. The dental practice is not signing up for a network or for any sort of contracted fee schedule.

Can our patients continue to come to us and will having the insurance penalize them or our office in any way?
  • Your patients can go to you or any other dentist without economic penalty since there is no network.
  • You may bill the patient for any part of the bill not covered by the insurance.

How much reimbursement or payment is received from the insurance company?
  • We reimburse based on the Usual and Customary schedule in effect for your zip code.
  • You can also call the insurance company to verify benefits by calling (877) 223-4693

How are claims filed?
  • Use the same process you do now. If you currently use a "clearing house" to electronically process your claims to the insurance company, continue to do so. Your clearing house will send the claim on to us. The electronic Payor ID is: CX015
  • If you prefer, to mail or fax the claim, the address is:
    * Group Link, Inc    PO Box 20593   Indianapolis, IN 46220
    * fax (317) 578-7312

How long does it take to receive payment?
  • Claims are normally paid in 2 weeks. We will send the check to your office if the claim form authorizes "assignment of benefits" otherwise to the patient.

Why is your brochure so general? Our patients have questions not addressed in your brochure.
  • We did not want to place your office in the position of being an "expert on dental insurance". The purpose of the brochure is to make your patients aware that dental insurance for individuals is available and provide a resource for additional information..our web site or toll free number.
  • Also, by limiting the brochure to general information, it does not create any potential liability for your office since you are not answering specific insurance questions.

What is the cost of insurance?
  • Our web site provides the exact cost in any zip code and for each of the several options available. Costs also vary by age. It is very affordable...single coverage begins as low as $23.16 per month in some areas. Family coverage for as little as $57.03.

Is coverage available for seniors?
  • Yes, also available on the DBfor1 web site or we can mail you a brochure.

Is there a waiting period before coverage begins?
  • Coverage begins immediately (1st of the month) for preventative care and Type II & III procedures. Your patient can also chose a policy that does have a waiting period for Type III coverage.

Where can our patients find out more?
  • Go to our web site: www.DBfor1.com
  • Call our toll free number (866) 907-7597 or (303) 531-8292
